Revive’s Personalized Men’s Health Subscription provides ongoing, individualized men’s health care under the guidance of Chase, PA-C. Your membership includes concierge access to Chase during practice hours, medication management, routine follow-up visits as clinically necessary, and routine laboratory orders and review to monitor treatment, safety, and progress.
When clinically appropriate, your individualized treatment plan may include personalized compounded injectable medication prescribed and coordinated through a licensed pharmacy. Treatment decisions and dosing are based on your medical history, symptoms, laboratory results, treatment response, tolerance, and ongoing clinical eligibility.
The subscription is $289 every 56 days and automatically renews every 56 days until canceled. Continued participation and treatment require compliance with clinically appropriate follow-up and laboratory monitoring. Subscription pricing is subject to change with at least 30 days’ notice.
TERMS: By enrolling, you authorize Revive Medical Wellness & Aesthetics to charge your payment method $289 every 56 days until canceled. You may cancel before your next scheduled renewal by contacting Revive Medical Wellness & Aesthetics. Cancellation prevents future renewal charges but does not automatically refund charges that have already been processed.
